Please allow this letter to serve as a formal request for my personnel records pursuant to ORS 652.750. I request my complete personnel records as defined by Oregon law, including but not limited to:
- all documents used to determine my qualification for employment, promotion, additional compensation or employment termination or other disciplinary action all documents signed by me relating to my job or my performance;
- all of my time and pay records for the past two (2) years (see 2016 update to ORS 652.750(1)(d));
- and any documents relating to dispute resolution procedures or timelines.
Pursuant to Oregon law, I have a right to certified copies of these records, whether or not the documents were placed in my official personnel file. Please provide copies of all documents requested above by email or by mail to: [NAME] [ADDRESS] [CITY, STATE, ZIP]
Please deliver my personnel records as soon as possible, but no later than [45 DAYS + 3 FOR MAILING], as required by Oregon law. I agree to pay any reasonable copying fees of not more than $20, but would prefer to receive documents in digital formats at less cost.
